研究意义
脑瘫是指由于出生前、出生时、婴儿早期某些原因造成的非进行性脑损伤所致的综合症,主要表现为中枢性运动障碍和姿势异常,可伴有智能落后及惊厥发作、行为异常、感觉障碍及其他异常。在发达国家平均每1000个新生儿中有1~3个患有脑瘫。在我国脑瘫患儿的发病率为2‰~2.5‰[1、2]。脑瘫常发生于新生儿期,是新生儿常见的致残性疾患之一,严重影响其日后的生活。随着医疗技术和诊疗手段的提高,许多早产儿、极低出生体重和重度窒息患儿被抢救存活,早期干预能有效地降低高危儿转变为脑瘫儿的发生率,提高人口质量。本课题研究早期干预改善和提高高危儿粗大运动的疗效,促进高危儿的生长运动发育,降低脑瘫的发生率。
国内外研究现状及发展趋势
虽然我国在脑瘫儿治疗和康复技术上取得较大进展,但对高危儿早期干预的措施未被重视和普及,以致脑瘫的发病率有上升趋势。目前研究表明,脑瘫的高危因素包括早产、宫内感染、宫内发育障碍及围生期窒息等,若能对高危儿进行早期诊断和干预,干预的有效率达到85%-95%[3、4]。
随着围产医学的进步,存在缺血缺氧性脑病、严重黄疸、极低出生体重、早产、重度窒息等高危因素的婴儿被抢救存活下来,而高危儿中CP患儿的发生率却极高,在目前的医学条件下,早期诊断CP患儿存在一定困难,因此早期干预和预防高危儿转化为CP患儿,降低CP发生率就显得极为重要。早期干预的主要目的在于促进正常运动功能的形成和发育,阻止异常姿势反射和异常肌张力的发展,建立和发展独立的日常生活能力,发展交流能力和应变能力,防止异常模式的固定化及挛缩畸形的发生和发展。在国外,AIMS已经被广泛地运用在临床,筛查识别出存在运动发育迟缓或异常,并且需要进行干预治疗的婴儿,尤其是对于可能发生运动异常的高危儿进行筛查[6、8-12],通常将AIMS总分对应的百分位低于5%或10%作为运动发育异常的判断标准[13、14],将Alberta量表广泛地运用到国内高危儿的早期识别和早期干预,有效地指导高危儿不同粗大运动发育水平干预是一个值得提倡和推广的方法。
